CAP/C (Community Alternative Program for Children) Home Care

Description of Services:   
The Community Alternatives Program for Children (CAP/C) provides cost- effective home care for medically fragile children who would otherwise require long-term hospital care or nursing facility care. Services include: case management, respite care, home and vehicle modifications, palliative care, hourly skilled nurse or nurse aid and many more.

Ages Served:  Birth up to age 21

Fees/Payment Type Accepted:   Medicaid

Eligibility: 
​Children who meet all of the following may be approved for CAP/C participation:
  • Age under 21 years
  • Medically fragile
  • At risk for institutionalization
  • Lives in a private residence while on the program
  • Requires one waiver service (excluding respite and waiver incontinence supplies) in addition to case management at least once per 90 days.
  • Requires the service to supplement, not replace, existing resources.
  • Requires the service to meet specific unmet needs of the child.
  • Is not requesting CAP/C solely to become eligible for Medicaid or other Medicaid services.
  • Meets the Medicaid eligibility criteria for CAP/C including approval for disability by the Social Security Administration if applicable.
  • Has an emergency back-up plan with adequate social support to meet the basic needs outlined in the CAP/C assessment to maintain their health, safety and well-being.
  • Can have his/her healthcare needs met within the CAP/C cost limit for the selected services and there is no other more conservative or less costly treatment available statewide.

Description of Services: 
Group Living-Summit administers two 6-bed residences for adults who have a developmental disability (the Lighthouse and the Ark).  The homes are staffed by trained and credentialed paraprofessionals 365 days a year.  The Group Living Program's mission is to provide quality supports for its residents by promoting health and safety, self-determination, community inclusion, and dignity and respect.  The stable home environment and caring staff support each person in achieving their life goals.
Vocational/Day Programming-At the Summit Training and Educational Program (S.T.E.P.), participants are engaged in acquiring basic life skills, broadening their academic knowledge base, and receiving pre-vocational training.  Participants also explore vocational interests, and they are offered opportunities and supports to start their own microbusinesses. Supported Community Living (SCL)-
This program provides support services to those individuals who live in the community but still need varying degrees of services to ensure that this arrangement is successful.  Services commonly address areas such as financial management, nutrition, medical needs coordination, maintenance of housekeeping standards, and coordination of transportation needs.  SCL also administers Alternative Family Living (AFL).  This living arrangement matches the consumer with a family who agreed to provide a home to the consumer, who ideally becomes an extended member of that family.

Fees: 
None if DSS approved or SSI recipient.  There is a cost for private pay.

Our goal is to help our members live independently, achieve their goals and be a successful part of his/her home and community. Community Alternatives Program for Disabled Adults (CAP/DA)

Description of Services: 
This program makes care at home a real possibility for many people who face nursing placement. CAP/DA exists to supplement rather than replace the formal and informal services and support already available to an individual. The program’s goal is to enable individuals to remain their homes or to return to their homes from nursing facilities and live as independently as possible. 


Ages Served:    Age 18 and older

Eligibility:

 To receive services:
  • Person must be 18 years of age and older
  • Eligible for Medicaid
  • Live in a private residence and is at risk of being placed in a nursing facility, or lives in a nursing facility and desires to return to a private residence
  • Requires nursing facility care (IC or SN) as determined through the Medicaid prior approval process
  • Needs CAP/DA services to remain safely at home
  •  Can have his or her health, safety, and well-being maintained at home within the Medicaid cost limit
  • Desires CAP/DA services instead of institutional care

Description of Services: 
The purpose of ASHE SCHOOLS OF HOME EDUCATION (ASHE) is to promote the common interests of ASHE members: to provide encouragement, support and guidance to its home schooling families living in Ashe County and surrounding areas. We are a home school organization and are governed by Christian principles; but, we are open to everyone who home schools, regardless of religion or race.

Membership:
​You must fill out a membership form and pay the annual membership fee.
First Semester- First   Tuesday    in September to Last Tuesday in November
Second Semester- First   Tuesday    in January to second week in May
-    All families participating in classes must register as a member and sign a waiver form before participating in classes. If you are new and would like to try a class before signing up you may do so, but a waiver must still be signed first. All class fees should be paid to each individual teacher. Checks for Membership and Facility Fees can be made to ASHE.
-    Each family MUST sign up for monitor duty 3 times during the semester. There are NO exceptions. If you are unable to do monitor duty you must find someone to take your turn as there is only ONE monitor per Tuesday. This is required so that we can provide adequate supervision for our children. In order for us to have the benefits of these classes we must all work together and do our part. There is a sign up sheet on the front desk.  All families participating in   Tuesday    classes are responsible to assist in keeping the facility clean and “as we found it” each week.
