Description of Services: Mountain Valley Hospice & Palliative Care provides hospice and palliative services to terminally ill patients and their families in 17 counties from 8 locations in North Carolina and Southern Virginia. These individualized services are designed to specifically address the needs of children and families as they face the challenge of coping with chronic or life-threatening illness. A team of experienced professionals can help guide parents and physicians to the most appropriate service. Mountain Valley Hospice & Palliative Care offers an inpatient/outpatient Palliative Care specialty consultation service for expert management of uncontrolled symptoms in patients with life-limiting illnesses which are associated with a life expectancy of 18 months or less. It is focused on providing patients with relief from the symptoms, pain, and stress of a serious illness – whatever the diagnosis. The goal is to improve quality of life for both the patient and their family.

Ages Served: Available to patients of any age, religion, race or illness.

Payment/Fees:  Medicare, Medicaid,  Most insurance companies, such as Blue Cross Blue Shield, Cigna, MedCost, etc.,   Mountain Valley Hospice and Palliative Care will never turn away a patient due to their lack of insurance or ability to pay.

Community Alternatives Program for Disabled Adults (CAP/DA)

Description of Services: This program makes care at home a real possibility for many people who face nursing placement. CAP/DA exists to supplement rather than replace the formal and informal services and support already available to an individual. The program’s goal is to enable individuals to remain their homes or to return to their homes from nursing facilities and live as independently as possible. 

Ages Served: Age 18 and older

Eligibility: To receive services:
  • Person must be 18 years of age and older
  • Eligible for Medicaid
  • Live in a private residence and is at risk of being placed in a nursing facility, or lives in a nursing facility and desires to return to a private residence
  • Requires nursing facility care (IC or SN) as determined through the Medicaid prior approval process
  • Needs CAP/DA services to remain safely at home
  •  Can have his or her health, safety, and well-being maintained at home within the Medicaid cost limit
  • Desires CAP/DA services instead of institutional care

Description of Services: Goals of the CAP/DA and CAP/C programs are to assist the elderly and disabled in leading lives in the community with dignity and independence to the greatest extent possible and to deliver necessary services in a cost-effective manner.
Home Health provides home visits that follow a customized therapeutic treatment plan that may include nursing, physical, occupational, and speech therapies, in-home aide, and medical social work services.

Ages Served: Available for disabled adults and children. This program provides assistance to families in keeping loved ones at home through social intervention, personal care, and light housekeeping. The program's goal is to allow adults and children needing long-term medical care to return to or remain in their homes.

Service Area: Wilkes County

Eligibility: Residents of Wilkes County who meet all of the following requirements: all ages but must meet the medical eligibility requirements:  at risk for institutionalization, and meet the Medicaid eligibility requirements.CAP/C is available to children from birth to 18 years of age, who are medically fragile. Individuals must require medical attention and/or support services to remain at home. Residents must reside in Wilkes County.
To qualify, individuals must meet medical, functional and financial eligibility requirements. Individuals must be at risk of nursing home placement, but must also be able to be safely cared for in the community.

CAP/C (Community Alternative Program for Children) Home Care

Description of Services: The Community Alternatives Program for Children (CAP/C) provides cost- effective home care for medically fragile children who would otherwise require long-term hospital care or nursing facility care. Services include: case management, respite care, home and vehicle modifications, palliative care, hourly skilled nurse or nurse aid and many more.

Ages Served: Birth up to age 21

Fees/Payment Type Accepted: Medicaid

Eligibility: Children who meet all of the following may be approved for CAP/C participation:
  • Age under 21 years
  • Medically fragile
  • At risk for institutionalization
  • Lives in a private residence while on the program
  • Requires one waiver service (excluding respite and waiver incontinence supplies) in addition to case management at least once per 90 days
  • Requires the service to supplement, not replace, existing resources
  • Requires the service to meet specific unmet needs of the child
  • Is not requesting CAP/C solely to become eligible for Medicaid or other Medicaid services
  • Meets the Medicaid eligibility criteria for CAP/C including approval for disability by the Social Security Administration if applicable
  • Has an emergency back-up plan with adequate social support to meet the basic needs outlined in the CAP/C assessment to maintain their health, safety and well-being
  • Can have his/her healthcare needs met within the CAP/C cost limit for the selected services and there is no other more conservative or less costly treatment available statewide
