This includes programs for before, during and after delivery of their baby, and for the infants as well. The Section develops medical guidance for health care providers and offers technical assistance, consultation and training for staff who provide health services to women in the areas of nutrition, nursing, social work, education and training. These programs and services improve the overall health status of women, increase planned pregnancies, reduce infant sickness and death, and strengthen families and communities. The Community Health Team Description of Services: The Community Health Team at AppHealthCare works with local stakeholders to identify, assess and address public health. We focus on community health outcomes by cultivating partnerships to strengthen public health through evidence-based prevention, intervention, systems level change and resiliency building.We envision a community where everyone has an equal opportunity to live healthy and productive lives. We work toward this vision in partnership with schools, community and faith leaders, health care professionals, state partners and other local stakeholders to identify health priorities and implement evidence-based strategies to address them. Mother-Baby Home Visiting Program Description of Services: How does the program work?At 3 weeks postpartum or at any point within the first 3 months after delivery, a nurse will visit your family at your home to check on mom, baby and the whole family. The visit includes:
The program also provides a meal for the family and gift bag.All Mother-Baby Home Visiting nurses are highly trained and are passionate about caring for families of newborns. They have many years of experience and are invested in supporting families as they welcome the newest members of their family. How do families register for the program?Families can complete the online registration form (forma en Española) and a member of the Mother-Baby Home Visiting team will reach out to schedule an appointment. The organization provides housing, nutrition, and prevention in a safe, nurturing and healthy environment through programs such as family housing, emergency shelter services, food box pantry and many more. Fees/ Payment Type Accepted: There is no fee to stay at the Hospitality House, but you must provide verification of homelessness. This may be an eviction notice, statement of a family member or a community member. Our mission is helping children enter school healthy and ready to succeed. APFC works towards this mission by seeking out agencies, organizations and individuals that work with young children and their families to create collaborations to enhance or create needed services. The Alleghany County Partnership for Children Board of Directors have adopted planned goals to assist the children and families in our community. OUR GOALS:
